3. Appointments and Online Forms
You may use the Site to request appointments or submit health-related inquiries. Submitting a form does not confirm an appointment until our staff confirms it. Giving us your phone number does not by itself sign you up for text messages; you choose whether to receive them, as described in section 4.
4. SMS/Text Message Program

Appointment and care messages. If you check the box agreeing to appointment text messages on one of our forms, or agree with our staff in person or by phone, you agree to receive text messages from American Medical Wellness about your care: appointment confirmations and reminders, scheduling updates, replies to your inquiries and follow-ups after your visit.
Marketing messages. Promotional messages, such as special offers and wellness tips, are sent only if you separately check the box agreeing to marketing text messages. You can receive appointment messages without agreeing to marketing messages.
Consent to receive text messages is not a condition of purchasing any product or service. Opt-in boxes are never pre-checked.
- Message frequency: varies, typically 1 to 4 messages per month, and more around a scheduled appointment.
- Cost: Message and data rates may apply.
- To opt out: reply STOP to any message at any time. You will receive one final message confirming that you have been unsubscribed.

- Carriers: wireless carriers are not liable for delayed or undelivered messages.
- Eligibility: you must be at least 18 years old and the account holder of the mobile number, or have the account holder's permission.
We handle your phone number and opt-in information as described in our Privacy Policy. We do not sell or share your mobile number or opt-in consent with third parties for their marketing purposes.
5. Intellectual Property
